To order Contacts:
· Call Dr. Stanley Saltzberg at 416 733 4444
· Ask to renew Andrew Rivkin’s Contact Prescription for 6 months
· Andrew would like 3 months supply of the old contact lens prescription and 3 months of the new contact lens prescription. The name on the new lens should be written in his file.
To Pick Up:
· Once ready the receptionist will call and let you know
· Pick up address 180 Sheppard Ave E- Postal provided in case you need to map quest it M2N 3A4
